Scheduling and Preparation for Visit
New infertility consultations are scheduled through the appointment schedulers by calling the main office phone number @ +1 (949) 845-1101. Fertility procedures may be performed in the office or at our out-patient surgery centers.
For vasectomy reversals, please call the number +1 (949) 592-0126 to schedule your free phone consultation. Patients may also call the main office or contact us by email to: Michelle Bernal @ mbernal@urologymedical.com.
All follow-up visits will be scheduled at the time of checkout. If you are not scheduled the main office can also take of your scheduling requirements.
If ordered tests are completed, you will need to meet with Dr. Spitz as a follow-up. If surgery is recommended, we will then contact you to proceed with scheduling.

Once you have made your initial appointment, you can download your patient forms from our Patient Forms page. Please print and complete the forms prior to your arrival, which will allow us to speed up your registration.
Insurance Coverage
We accept most insurance plans. Please call us to verify whether or not we accept a particular insurance company's coverage.
